Welcome to the Support at Home Program

Support at Home is the new Australian Government in-home aged care program which commenced on November 1st, 2025, replacing the previous 'home care program'. The new system brings together existing services with simplified budgets, equitable pricing and clearer assessment levels.

What is the Support at Home Program?

Support at Home brings together existing services with simplified budgets, equitable pricing and clearer assessment levels. It provides ongoing support as well as specialised pathways including:

  • Restorative Care Pathway for short-term allied health support to improve independence
  • Assistive Technology & Home Modifications (AT-HM) scheme to fund equipment and home safety adjustments
  • End-of-Life Pathway for up to three months of specialised care at home

The program establishes the Single Assessment System to make it easier for older people to access aged care services as their needs change.

Services Available Under Support at Home

The Support at Home program covers a comprehensive range of services designed to help you maintain your independence and quality of life at home.

Personal Care

Assistance with daily personal care activities including bathing, dressing, and grooming

Domestic Assistance

Help with household tasks, cleaning, laundry, and meal preparation

Social Support

Companionship services and assistance with social activities and community participation

Transport Services

Assistance with transport to appointments, shopping, and social activities

Nursing Care

Professional nursing services and health monitoring in your home

Allied Health

Access to physiotherapy, occupational therapy, and other allied health services

How to Enter the Support at Home Program

We have established the Single Assessment System to make it easier for older people to access aged care services as their needs change.

Check Your Eligibility

If you are not already receiving services through My Aged Care, the first step is to check your eligibility for an assessment. You can do this online at My Aged Care, or by calling 1800 200 422.

Assessment Process

If your application is successful, you will be referred for an assessment. Assessments are done in person, at your own home.

Receive Your Support Plan

When assessed as eligible for the Support at Home program, you will receive a Notice of Decision letter and an individual support plan to share with your provider. This will contain:

  • A summary of your aged care needs and goals
  • A classification with an associated ongoing quarterly budget and a list of your approved services; and/or
  • An approval for short-term supports, which may include funding for assistive technology, home modifications, Restorative Care Pathway, or End-of-Life Pathway

Classifications and Budgets

Your Support at Home classification is determined during your aged care assessment and sets your quarterly budget based on your needs.

How Classifications Work

Your assessment sets your classification for ongoing services and approval for short-term pathways.

Your assessment also sets your quarterly (3-monthly) budget, based on your needs.

If you don't spend your budget each quarter, you can carry over unspent funds up to $1,000 or 10% (whichever is greater) to the next quarter.

Assistive technology and home modifications through the AT-HM scheme are not budgeted quarterly. If approved for the AT-HM scheme, you will have a funding level for a longer time.

*Quarterly budgets and annual amounts are effective from November 1st, 2025 and are subject to change in March and September each year in line with indexation.

Am I Eligible for Support at Home?

To be eligible for the Support at Home program, you generally need to meet the following criteria:

  • Be 65 years or older (or 50+ for Aboriginal and Torres Strait Islander people)
  • Have been assessed as needing support to remain living at home
  • Be an Australian citizen or permanent resident
  • Live in an area where Support at Home services are available
  • Meet the income and asset test requirements

'No Worse Off' Principle

If you were already receiving a Home Care Package, on the National Priority System or assessed as eligible for a package on or before 12 September 2024, you will be no worse off under Support at Home.

Contributions and Care Management

Understanding your financial contributions and how care management works under the Support at Home program.

Your Contributions

Contributions are an essential part of the Support at Home program, to ensure a fairer system that can provide better and more tailored services in an affordable way.

You will only pay contributions for services you receive.

The government will fully fund clinical care, such as nursing and physiotherapy. You will not be required to contribute for these services.

Standard Contribution Rates (from November 1st, 2025)
Clinical care:0% (fully funded)
Independence support:5% - 50%*
Everyday living:17.5% - 80%*

*Depending on age, pension status and income/assets

Care Management

You will have access to care management, which supports you to get the best outcomes from your aged care services.

If you receive ongoing services, 10% of your budget each quarter will go to your provider for care management. This amount will be the same even if you have decided to self-manage all or some of your services.

Care Management Activities Include:
  • Care planning and service coordination
  • Budget management and oversight
  • Monitoring and responding to changing needs
  • Supporting informed decision making
  • Health promotion and education
  • System navigation and linkage
  • Problem-solving and risk management

'No Worse Off' Principle for Existing Clients

If you were already receiving a Home Care Package, on the National Priority System or assessed as eligible for a package on or before 12 September 2024, you will be no worse off under Support at Home.
